Referral Process for MSP-Covered Consultations
A Midwife, Physician, Registered Nurse, Nurse Practitioner or Registered Social Worker
may make a referral for families with babies under 6 weeks old for MSP-covered care.
1. Complete the PDF referral form and fax it to our office at 778-309-7931 OR Complete the online intake form and submit it through the website.
2. Advise the patient to use this website to book their own appointment. If no appointment gets booked our office will call to offer assistance with booking, or return the referral to you if patient declines an appointment.
